 Stewart Community Home provides a safe and supportive environment for homeless individuals with mental and physical disabilities. Our mission is to offer not just shelter, but a community where residents can thrive and rebuild their lives.

Initial Application Process:

1. Fill Out the Form: Complete the initial application form below with your basic personal and contact information.
2. Review: Our admissions team will review your submission to ensure it meets our basic eligibility criteria.
3. Follow-Up: We will contact you to request additional information and schedule a meeting if you meet our initial criteria.

Anyone who wishes to apply for residency must make an appointment with our Med Room Supervisor, Regina Blount, after submitting this application. This can be done by an individual on their own or through a referring agency. Call (706) 507-7040 for admissions information.
